HBPM có khả năng đo lặp lại cao hơn đo HA tại phòng khám, khả năng dự đoán biến chứng cơ quan đích, kết cục tim mạch, tỉ lệ tử vong tốt hơn đo HA tại phòng khám, đo lường sự thay đổi HA hàng ngày, cải thiện tuân thủ điều trị, triệt tiêu được hiệu ứng áo choàng trắng, giá trị của nó thấp hơn giá trị đo HA phòng khám. Ngưỡng tăng HA tại nhà là HATT ≥ 135 mmHg và hoặc HATTR ≥ 85 mmHg và ngưỡng mục tiêu hạ HA là <130/80 mmHg. Nên theo dõi HA tại nhà 7 ngày và không dưới 3 ngày với các phép đo trùng lặp cách nhau 1 phút vào buổi sáng (trước khi dùng thuốc nếu đang điều trị) và buổi tối, giúp xác định THA áo choàng trắng và THA ẩn dấu (I,B), HBPM được khuyến nghị để dự báo nguy cơ tim mạch và giá trị tiên lượng tốt hơn HA phòng khám (II, B) [1]
Theo dõi huyết áp lưu động (ABPM)
ABPM cung cấp nhiều nhiều chỉ số HA trong điều kiện hoạt động thông thường hàng ngày và giấc ngủ, có mối liên hệ chặt chẽ hơn trong dự đoán tổn thương cơ quan đích, kết cục tim mạch và tử vong. Nó còn giúp khả năng phân biệt giữa THA và THA kháng trị thực sự, xác định độ biến thiên HA trong 24 giờ và sự tăng vọt HA vào buổi sáng có giá trị tiên lượng bất lợi, độc lập với HA trung bình 24 giờ , lợi thế quan trọng nhất và gần như duy nhất lượng giá HA ban đêm không giảm, mà tăng HA về đêm làm tăng nguy cơ rõ rệt biến cố tim mạch.
